Beacon Project Update

Beacon Community Program

Groundbreaking telemonitoring pilot underway!

As part of the Beacon Project, HEALTHeLINK recently launched a diabetes telemonitoring pilot program to focus on helping physicians improve primary and specialty care for diabetic patients. Goals of the program include reducing preventable emergency room visits and hospital re-admissions.

Approximately 150 diabetic patients are currently participating in the telemonitoring pilot. Using mobile devices, blood pressure, glucose readings and other vital signs are reported electronically to health care providers through HEALTHeLINK. Catholic Health’s McAuley Seton Home Care and Kaleida Health’s Visiting Nurses Association of Western New York were hired to support the telemonitoring services with home interventions with the patients when needed. Nurses and other health care professionals are charged with interpreting the readings and reporting the health information that is critical for the treating physician to see. Ultimately, relevant data captured by the patient in their home will be transmitted through HEALTHeLINK to the treating doctor’s EMR!

One Click Could Save Your Life... Consent Campaign in Progress

A major advertising campaign is now underway to encourage consumers to provide their consent. Using the theme of “this could save your life,” the campaign features a pen to illustrate that your signature is all you need to take control of your health care. When you say “yes” to HEALTHeLINK, you give doctors involved in your care fast secure access to your medical history – everything from hospital visit summaries to test results to diabetes medications. Having your medical records available helps doctors treat you better, and, in an emergency, this information could be lifesaving. The campaign includes billboards, online and Facebook ads and spots on various local radio stations.

To date more than 300,000 Western New Yorkers have provided their HEALTHeLINK consent. Have you signed yours yet?

New method for authenticating users

The HEALTHeCOMMUNITY portal now has an additional method for authenticating users. In addition to the text message and phone voice delivery of the one-time-password for logging into the portal, users now have the option of carrying a token/fob that generates the one-time-passcode. This eliminates the need for using your cell phone or office phone during the log-in process and for many users will be a more convenient method for logging into the portal.

Digital Image Exchange

This new service provides authorized users of HEALTHeLINK with access to diagnostic quality radiology images from multiple picture archiving and communication systems (PACS). No more lost images, films or CDs. Patients receive the most effective and efficient care because their treating physicians have immediate access to radiology images, as well as radiology reports. This service also improves patient safety by helping eliminate radiation exposure associated with duplicate exams.
