Beacon Project Update

Beacon Community Program

Please join us in welcoming Bob Hoover as Beacon Program Director and Gary Kerl as Beacon Project Director to our team. Bob will be responsible for the overall Beacon program direction, delivery and administration while Gary will primarily focus upon technology delivery in support of the Beacon project. Thank you to Todd Norris for his tremendous efforts on the WNY Beacon Project over the last year and a half. Everyone at HEALTHeLINK wishes him well in his future endeavors as he relocates to Florida to be closer to his family.

Work continues on the Beacon Project with a central goal for 2012 and the rest of the program on finishing strong. One of the main pilot projects underway within the Beacon is patient registries, which offer an innovative tool to focus on improving quality of care, measuring progress and providing real-time information on patients. As more providers adopt EHR systems, there are greater opportunities to use the patient data for population-based care of chronic diseases, such as diabetes, and identify quality improvement initiatives.

To measure the impact that our clinical transformation projects have on the community, the WNY Beacon team has implemented a comprehensive registry report that uses data collected from participating practices. The registry report consists of demographic clinical information for patients diagnosed with diabetes. The compiled data is de-identified and provides a set of distinct indicators that enable us to measure the impact of the changes that take place within a practice as well as across Western New York. Providing better access to digital reports

HEALTHeLINK recently entered into a partnership with Rochester-based eHealth Global Technologies and OptumInsight to deploy an innovative medical image exchange service that will enable faster diagnosis and treatment, improved outcomes, reduced radiation exposure and health care cost reductions. Over the next couple of months, participating physicians will have quick and secure access to quality diagnostic radiology and cardiology images through HEALTHeLINK. Surescripts adds CVS pharmacy data to medication history query

Participating physicians will now be able to access pharmacy data from local CVS pharmacies in the medication history query. Surescripts recently added this data to HEALTHeLINK. Patient prescription fill data at retail pharmacies is also available from Walgreens, Rite Aid and Target and also all claims data from pharmacy benefit managers.

infoTech Niagara’s 2012 BETA Award Finalist

We are pleased to announce that HEALTHeLINK has been chosen as a finalist for infoTech Niagara’s 2012 BETA Awards in the Health IT category! infoTech Niagara is “the” trade association for Western New York’s information technology industry. Winners will be announced on April 3. For a complete list of this year’s finalists, click here

Patient-Centered Medical Home Certification

In September 2009, New York State awarded HEALTHeLINK with a HEAL 10 grant to focus on improving patient care coordination and management through Patient-Centered Medical Home. By building on current health information and using improved clinical processes, primary care practices are able to pull labs, radiology and hospital results and then integrate these results with electronic patient data generated from the practice to provide well-informed diagnoses and care coordination for patients. Additionally, the practice will have a clearer line to those patients who are in greatest need of care but who may not actively seek care.

The original goal of the project was to assist 15 practices in achieving their Patient-Centered Medical Home certification from the National Committee for Quality Assurance through the use of HEALTHeLINK. To date, we have supported 54 practice sites. The most recent practices to receive their certification are Community Health Center of Buffalo, Jericho Road Family Practice (Genesee Street) and The Resource Center.

HEALTHeLINK is tracking to the Stage 2 Meaningful Use requirements

The U.S. Department of Health & Human Services recently announced the proposed Stage 2 Meaningful Use incentive rules regarding electronic health records. HEALTHeLINK and the various technical services that is provides to its participants is on track to help practices and hospitals meet the Stage 2 requirements.  The rules, which include standards-based interoperability and patient engagement, are expected to be finalized later this year and take effect in 2014. More information will be made available once the rules are finalized.

Grants available for practices that achieve recognition for diabetes care

The New York State Health Foundation is offering grant awards on a rolling basis for practices that achieve National Committee for Quality Assurance (NCQA) or Bridges to Excellence (BTE) recognition for diabetes care. The grant awards up to $2,500 per physician who achieves NCQA or BTE diabetes recognition (up to 10 physicians per practice) and will give an initial payment of 20% upon grant approval and 80% when recognition is achieved. Small practices (10 clinicians or less) can apply directly through the New York State Health Foundation here. Consent Campaign Update

At the end of 2011, HEALTHeLINK wrapped a major advertising campaign encouraging consumers to provide their consent. More than 350,000 Western New Yorkers have signed their HEALTHeLINK consent form to date, including approximately 160,000 in 2011 alone – more than double what was captured in the previous two years combined. In addition a remarkable 95% have said yes to have their health information shared through HEALTHeLINK. For more details or to download a consent form, click here.
