Beacon Project Update

Beacon Community Program

The Western New York Beacon submitted its annual report to the Office of the National Coordinator for Health IT earlier this year. To learn more about the project to date and to hear firsthand from several participating providers and patients, click here.

Over the course of the three-year project – which runs through the end of March – WNY Beacon has been able to build on HEALTHeLINK’s foundation by expanding the amount of data available within the health information exchange (HIE) and accelerating and facilitating the effective use of health IT. The project has also strengthened HEALTHeLINK through the addition of more data sources and the development of new functionality to increase the value of the HIE to users.

Here's a snapshot of a few of the major milestone WNY Beacon has achieved over the past three years.

- Connected 3 long term care facilities to the HIE to allow for better transitions of care coordination among patients and developed interfaces with 3 home health care agencies.

- In 2012, care coordination and care transitions made more efficient by over 188,000 EMR to EMR interoperable patient data exchanges.

- Approximately 95% of laboratory results and 85% of radiology reports generated in WNY are now available through HEALTHeLINK.

- Working with Catholic Medical Partners and P2 Collaborative of WNY, assisted with the clinical transformation of nearly 100 practices by developing workflows that use health IT to improve practice performance in terms of both quality and costs

- Implemented registries at 78 practices, representing 57,000 diabetic patients to track lab values, vitals and necessary tests, and to generate reminders and guidance for patients’ care

- Worked with home health and visiting nurse organizations to deploy telemonitoring devices in the homes of high-risk diabetes patients that allow patients to regularly submit glucose, blood pressure, and weight readings. Approximately 100 patients are enrolled and many are seeing improved health benefits and able to take better control of their diabetes.

Care Coordination Networks Technology Support

Through the New York State Medicaid program, Health Homes are being established statewide to enable all of an individual’s caregivers to communicate with one another so that the patient’s needs are addressed in a comprehensive manner. By leveraging its technical capabilities, HEALTHeLINK is currently working with three Health Homes - Greater Buffalo United Accountable Healthcare Network, Health Home Partners of WNY and Niagara Falls Memorial Medical Center Health Home - to provide a single community-wide solution to the coordination of patient care and bidirectional exchange of health information through Western New York’s clinical information exchange.

HEALTHeLINK continues its work to achieve further interoperability and interconnectivity among Health Home participants. Solutions are also being developed to accommodate organizations such as social care agencies and community based organizations that do not typically have electronic health record systems to enable them to communicate among other Health Home partners for the betterment of patient care. One example of this is the establishment of a Healthcare Internet Service Provider (HISP) to enable providers to send messages and attachments over a secure network. Among the future goals for Health Homes is the ability for the Care Coordinator to receive an alert when his/her patient has been discharged from or admitted to a local hospital in order to coordinate the appropriate services the patient will need upon discharge.

While HEALTHeLINK’s current care coordination efforts are focused on Health Homes, there are other accountable care organizations that can benefit from utilizing this technology for their specific care management solutions. In Western New York, there is no need to go out and build this technological infrastructure from the ground up as HEALTHeLINK can be leveraged for improved communication and sharing of health information among treating providers. Accessing Patient Records from the Department of Veterans' Affairs now available through HEALTHeLINK

HEALTHeLINK VHR users can now access information from The Department of Verterans' Affairs. The patient information contained may include:

  • Demographics
  • Vital Signs
  • Allergies
  • Medications - Prescription/Non-prescription
  • Problems/Conditions
  • Lab Results - Chemistry and Hematology
  • Immunizations
  • History of Encounters
  • Healthcare Providers

HEALTHeLINK's website goes mobile

HEALTHeLINK recently contracted with its development partner, Algonquin Studios, to build in technology known as “responsive web design” to www.wnyhealthelink.com to make the site’s content easier to read and interact with on today’s mobile devices such as tablets and smartphones. This newly responsive site will play a critical role in keeping participating practice information up-to-date for patients considering granting consent to allow their treating physician access to important critical information through HEALTHeLINK. Check it out on your smartphone and see how our website has gone mobile!

New long term care data source added

The Brothers of Mercy Nursing and Rehabilitation Center in Clarence is the third long term care facility to begin supplying clinical data securely and electronically to HEALTHeLINK. With authorized patient consent, treating physicians can now access Brothers of Mercy’s admission, discharge, and transfer notices to improve transitions of care.

HEALTHeLINK named a Finalist

HEALTHeLINK is named a finalist for InfoTech Niagara’s 2013 Beta Award in Health IT. This award is presented to the Western New York firm that brings unique and innovative products or services to the medical/health market, showing promise of major economic growth. We look forward to finding out the winner at the Beta Awards dinner on Wednesday, March 27.
