Beacon Project Update

Beacon Community Program

As one of the 17 Beacon Communities, HEALTHeLINK and its clinical transformation partners Catholic Medical Partners and P2 Collaborative of WNY, continue to build and strengthen the local health IT infrastructure by testing innovative approaches to make measurable improvements in health, care, and costs.

One pilot project currently underway by WNY Beacon is helping to improve communication when it comes to medications. WNY Beacon has partnered with Meducation, a standalone web-based tool which generates patient centered medical instructions that are easier to read, easier to understand and in the patients’ preferred language. Medication instructions are currently available in 17 languages (including English) for non-English speakers. In addition, 4 languages (Burmese, Karen, Somali, and Swahili) were added and funded by WNY Beacon to reflect the refugee populations in our region. The instructions also feature universal images depicting morning, noon, evening, and bedtime to illustrate the time of day when the medication should be taken.

Practices currently participating in the project include: Black Rock Pharmacy, Community Health Center of Buffalo, Elmwood Health Center, Erie County Department of Health, Jericho Road Family Practice, Mount St. Mary’s Neighborhood Health Center, Our Lady of Victory Family Care Center and Parkview Health Services. Since launching at the start of 2012, more than 200 medical instructions have been printed by the participating pilot practices to date.

First long term care facility joins HEALTHeLINK

Briody Health Care Facility LLC is the first long term care facility to join HEALTHeLINK as a data source. The facility has begun submitting admission, discharge and transfer notices. Located in Lockport, Briody Health Care Facility is the oldest family owned and operated nursing facility in Western New York. Connecting long term care facilities to HEALTHeLINK and Western New York’s clinical information exchange is also an important component of the work being done in conjunction with the Western New York Beacon Project. A portion of the $16.1 million grant awarded to HEALTHeLINK and Western New York by the Office of the National Coordinator for Health IT is being used to connect additional data sources, such as rural hospitals, home care agencies, and long term care facilities to expand the amount of data available to participating providers through the health information exchange.

By the Numbers

Here’s a brief snapshot of milestones reached to date!

  • Almost 400,000 patients have signed consent forms, with 95% saying yes.
  • 650% increase in number of patient record look-ups through the virtual health record from 2010 to 2011
  • 90% of laboratory results and 80% of radiology reports generated in WNY are available through HEALTHeLINK


  • 51% of pharmacy scripts (above the national average) were e-prescribed in 2011 vs. 17% in 2009


  • Assisted 76 practices in achieving Patient Centered Medical Home certification vs. original goal of 15
  • 3 home health care agencies involved in telemonitoring through WNY Beacon

AMDIS Award Recognition

Later this month, HEALTHeLINK will receive a 2012 award for outstanding achievement in applied medical informatics by the Association of Medical Directors of Information Systems. HEALTHeLINK is the only health information exchange to be recognized by the association this year!

EMR Adoption Program to end application process July 1

The funding effort between HEALTHeLINK and the region’s three major health insurers – BlueCross Blue Shield of WNY, Independent Health and Univera Healthcare – to assist primary care, OB/GYNs and pediatric practices in the purchase and implementation of an EMR system will end later this year. Applications will be accepted through July 1.

Since launching the program in 2010, 82 practices, which include 220 clinicians, have received a total of $450,000 in funding to acquire and implement EMR systems. In addition to the funding, HEALTHeLINK has provided approximately 30 hours of selection consulting and 100 hours of consulting services to participating practices to help transform their processes and workflow in order to ensure a smooth transition from paper to electronic medical records. This program has given practices the confidence they need to make the switch from paper to electronic records, one step at a time.

HEAL 17 Update

HEALTHeLINK is at the forefront in increasing the number of behavioral health providers using EMRs and improving communication between behavioral health and primary care providers for approximately 20,000 Western New York patients. Through HEAL 17, HEALTHeLINK was awarded a $3.8 million grant in 2010 from New York State. Work is currently underway to develop a model to support care coordination, with the primary care practice, which has been found to significantly reduce depression impairment and also improve cost-effectiveness compared with typical care. This model would integrate the primary care physician and his/her care coordination team with the behavioral health provider to provide a bidirectional interface to track and monitor behavioral health patient referral outcomes as well as to provide education consulting to the primary care practice. Clinical decision support tools will also be introduced to enable the practice to be more proactive by having current patient data and alerts at the point of care.

Due to the sensitive nature of this health information, it would not be available to other treating physicians through the virtual health record, only between the primary care and behavioral health providers. Other components include the implementation of telemedicine to provide real time video consultations between patient and provider and provider to provider. In addition, a care coordinator would be assigned to the patient for continued follow up and education. The pilot project is expected to launch with selected primary care and behavioral health participants later this year.

Results Delivery      

Through HEALTHeLINK, nearly 1,000 physicians are currently receiving automated delivery of their patients’ test results, transcribed reports and radiology services from the actual source of data directly into their EMR through results delivery. Results delivery improves operational efficiency by eliminating the need to import paper documents into one’s EMR. Participating providers will receive the results as long as they are listed on the result as one of the following: attending, ordering, referring, consulting, or copied to. EMR to EMR Exchange

Currently more than 500 physicians, who have Medent as their EMR vendor, are able to effectively communicate electronically by sending patient information directly from their EMR to another treating doctor’s EMR for referral purposes using a Continuity of Care Document or CCD This functionality improves care coordination, clinical efficiencies and practice workflow. We continue to work with eClinicalWorks, Allscripts and other vendors to further this interoperability functionality.
