Access to Patient Data

Patients can contact HEALTHeLINK to request copies of their medical records through a secure, electronic method, or through postal mail. Depending on the amount of data and the delivery method (electronic or paper), the response time may vary. HEALTHeLINK will make every effort to respond to these requests in a timely and efficient manner. 

Patients can also contact HEALTHeLINK to request a List of Disclosures (aka Audit Log) of which HEALTHeLINK participant has accessed your records. 

To submit a request, please contact us through our Information Line or call us at (716) 206-0993 option 1 during our normal business hours (Monday through Friday 8:30am-5:00pm ET) to guide you through the process. 

Patients are responsible for protecting the privacy and confidentiality of their personal health information (PHI) once it is delivered to their choice of destination. The HIPAA law does not apply to individuals, or most patient health record systems (PHRs) and personal apps. We encourage patients to be mindful of where and to whom PHI is shared.

If you believe the data in HEALTHeLINK is incorrect in any way, please contact the data supplier (e.g., physician office, etc.) that collected that data from you, or contact HEALTHeLINK and we will provide you with contact information for that data supplier.
