CCD Exchange Participants

At the completion of a patient encounter or patient office visit, some practices upload a CCD (Continuity of Care Document) that contains clinical and demographic information gathered in a current or a previous encounter at the practice. This information may include: Demographic Information, Allergies, Immunizations, Medications, Problems List, Lab Results and a history of Procedures/encounters. Entries indicated as 42 CFR Part 2 Facilities upload data to HEALTHeLINK that is subject to the Federal Regulation 42 CFR Part 2 (certain providers of alcohol or drug abuse care).
